Fees
As a patient of ARA, there are certain fees associated with the services we provide. ARA physicians participate with Medicare and several other insurance plans. Participation varies as changes occur within the insurance industry. Please check with our receptionists to confirm if we participate with your insurance plan.
Cost-share responsibility:
Your insurance may require you to share the cost of your medical services at ARA through a copay, coinsurance, or deductible. Please be aware that ARA is considered a specialist office, so if you have a deductible, it will apply to the services you receive.
For your convenience, we will file your primary insurance claims if you provide us with the correct billing information. We ask you to pay the deductible, co-insurance, or co-payment at the time of service. We accept Visa, Mastercard, American Express, Discover, cash, and checks.
Online Bill Pay
Phreesia Online Payments gives you the flexibility to pay your balances at home or on the go. Online Payments is a quick, easy, and secure solution for submitting payments.
Self-pay:
ARA offers a self-pay rate to patients who have no health insurance or have non-participating health insurance. The self-pay amount owed is expected to be paid in full at the time of service.
No-show and late cancellation fee:
ARA requests that cancellations or scheduling changes be made during clinic hours no later than the last business day prior to your appointment. If you are a new patient to our practice, the missed appointment fee is $200. If you are an established patient of the practice, the missed appointment fee is $50. In order to reschedule your missed appointment, you will be required to pay the missed appointment fee. Appointment changes, including rescheduling or canceling an appointment must occur during ARA’s business hours, which are Monday through Thursday, 7:30 am to 5:30 pm. If an appointment is canceled after hours for the next business day, it will be subject to a cancellation fee.
Fees are assessed for the following reasons:
- Same-day cancellation of an appointment
- Cancellation of an appointment after hours on ARA’s business day before the appointment (this includes appointment reminder cancellations, messages left on voicemails, or with the answering service).
- Failure to arrive to a scheduled appointment (no-show)
Patients will be unable to schedule a new appointment until payment is made. Additionally, ARA reserves the right to cancel future appointments if payment is not made in advance.
Our providers’ appointment schedules are full. When an appointment time is reserved specifically for you and we are not given appropriate notice of your cancellation, we are unable to offer another patient the opportunity to be scheduled. We understand that emergencies arise, and therefore if the appointment is canceled in advance, you will not be charged.
Please note that this policy applies to all ARA locations and services.
Your provider is not involved in administrative policies and enforcement and will be unable to waive these charges for you.

Non-Sufficient Funds (NSF) Policy:
A $50 NSF fee will be added to any patient’s account that is returned by our bank for non-sufficient funds.
Bank Dispute/Chargeback Policy
A $25 chargeback fee will be added to any patient’s account that erroneously disputes a credit card payment without first contacting the Business Office for dispute resolution. The chargeback fee is to cover bank processing fees associated with the dispute.
